For example a poor quality weapon may misfire and/or break

for example:A rocket launcher, it explodes with the ammo inside causing the explosion. leaving one dead person and a few pieces of a broken rocket launcher. To counter this, maintenance by someone who has decent crafting could prevent or reduce the chance of this. Would be amusing for mortars to explode in a raider's base.

Would have to be rare though, say up to 10% in a poorly maintained weapon, and possibly related to the AI chosen.

This happened occasionally in farcry2, and while it was never directly "bam you dead", it was funny to occasionally see guns simply break, or a rocket misfiring and flying around in circles. I think similar effects would be nice, IF they also effect raiders...like an enemy with a mini-gun about to mow down your colonist, but the gun ripping apart before it fires. So I support this idea, so long as its never just full on killing the colonist, but dud grenades, breaking guns, and wonky firing missles would be funny.
Why to people worry about following their heart? Its lodged in your chest, you won't accidentally leave it behind.

Its bad because reasons, and if you don't know the reasons, you are horrible. You cannot ask what the reasons are or else you doubt it. But the reasons are irrefutable. Logic.
