Does where you craft something have any impact on its quality? or is it only determined by crafter/builder skill? For ex, if i build a chair in the dark, in a dirty room, or outside, does it have a lower chance of higher quality?

Only the skill of the person finishing the item is taken into account when deciding quality.
I believe it's simply skill when it comes to crafting and item quality.
I do know cleanliness and lighting effects research speed, medical treatment quality, and food safety.
Temperature and Inside/Outside also have an effect on craft-speed. I am not sure about quality.
As it was told - only skill.

my lately become bad back old 20-on-crafting colonist seems to have become less valuable in quality of his craftings. Same observation when he is high on smokeleaf or alcool: quite lower quality on his manufacts. May be it is simple randomness..you really can't say.
And do bionic eyes and harms count for quality? or just for the speed in execution?
What i could observe is against the "only skill"
When you think your lvl 20 crafter should craft more legendary, then you need to get a mod (Static Quality Plus).
Without mod, there is still just a small chance to craft legendary's. He still craft more normal/good then master/legendary. Thats the reason why master/legendary raise at the tradevalue so much, because they are that rare.

I would prefer if it was a mixture between surgery and cooking :
A chance to botch the construction, supported by surroundings and difficulty of the crafted item
