I admit I have many mods but all of them are qol and shouldnt interfere with world generation...
I started a new game and found this. Ancient danger automatically opened, ready to fight me:

DrakeFS has the same bug with a save. [1.1] Grand Stele replaced a wall of a ancient evil (https://ludeon.com/forums/index.php?topic=50381)
His settings (seed: arthur, coverage 30%, 1.79°N 2.60°W, everything else default) generate an ancient danger breached by steles in unmodded 1.1.2575.
