Not sure if this is a bug or not.  I suppose it could be a hard-mode feature.  ;)

Circumstances:  Generating a new world

What Happened:  The Ancient Tomb room spawned with a Caterpillar inside.  The walls of the Room overlapped a Steam Geyser creating a gap in the wall.  This allowed the Caterpillar to immediately escape.  From the screenshot, you can see my one colonist about to have a very bad day and he's only a few hours into day 1.  (nevermind that I "may" have been trying to kill the other 2 already...)

Expected to Happen:  The Ancient Tomb room should have no gaps allowing any defenders out at the start of the game.

Reproducing:  I'm not sure, unless by sheer luck it happened again.  I included the world save file.  The map save file compressed was 987KB which is too big per the Attachment Restrictions.  I had 2 colonies right near each other on the map at coords:  56.53E, 26.67N  or  59.73E, 32.53N.  I think the 56.53E is the correct colony for this error.

If it makes a difference:
-The World was generated using the 9d build (0.9.725 i think)
-The Colony Map was generated using the 9e build (0.9.727)

Thanks!
