So currently removing an infected body part requires medicine, herbal or otherwise to queue the operation.  this means that: colonists dying of nose infections cannot simply have their nose chopped off with a kitchen knife to save them.  this is paradoxical because infections are more likely to happen in medicineless colonies.
One alternative would be an operation called: Chop out/Chop off (depending on body part in question) Chopping out would require no medicine but have a high chance of failure, blood loss, and repeat infection.  This would allow us to take a lesson from the American civil war:  When no medicine is available, use the rusty hacksaw.

I think would be a great idea.  Have two options, harvest or remove/amputate.  Medicine for harvesting and amputation requires none.  Simple, but let's have it only for infected, destroyed, and crushed body parts so it doesn't clutter the operation menu.

I'd be up for this, so long as it had serious consequences - your colonist is going to be in pain, they amputation is not going to be neat, and there's a chance of a secondary infection.

IMO medicine should enhance chance of success and stop mood decreases, while each body part has an extraction difficulty.
Arms yeah you can lob them off. Eyes? No so much. Organs? Fat chance.

Anesthetic will prevent pain, and in doing so keep mood. But pain can incap and cause colonists to go mad. Also there should be a debuff for the empaths and sensitive people who hear the screams of anguish from the people in the surgery rooms. Unless sound proof. Doc still takes the psych hit though.

I would keep the obligation.

We can use Herbals medicine to operate now, meaning you can rarely actually run out of medicine to operate as long as you played well and store those in case of problem.

Keeping one last dose of medicine in case of needed amputation is part of the gameplay.

I would eliminate entirely the requirement for medicine in order to operate and instead add an option to only operate with specific types of medicine: closest, glimmerworld meds, regular meds, herbal meds, none.  Obviously chance of infection would vary accordingly.
