In the Operation tab, there should be a checkbox (enabled by default):

"Don't operate until room is clean"
I find it highly frustrating to draft colonists, wait until a piece of dirt becomes cleanable, then clean it and proceed operating. Operation success chance depends on room cleanliness. Even with wound treatment you don't get a second chance to treat someone. But with operation, you're often spending expensive resources and failures hurt.

If "Don't operate until clean" is checked on, the doctor will first try to clean the room and only then start operating. If the doctor is a jerk and unable to clean, he will wait inside until (hopefully) another colonist comes by and cleans the hospital.

By the way... it's natural to assume better medicine improves chance of success of operation. But doctors don't seem to prefer Medicine over Herbal Medicine, and may or may not randomly pick Glitterworld Medicine for operation. What's the truth ?

This would be an excellent addition to Rimworld's medical system.
