Like i let my colonist drink beer under the "social drugs usage" policy. But apperantly beer is so harmfull that in a year or so literally everyone has cirrhosis.

The pirates do not have enough livers to sell me to keep up with this shit.

Maybe we had bad luck. If not, drug-related illness happens far to often.
It's hard to believe and makes using drugs too unattractive.

Guess what happens if one grabs a beer every day.

Ive been wondering about this. I haven't had the issue except with folks who had chemical seeking traits, but I'm not sure if I'm just getting lucky or what. I have it set to social drugs but they don't seem to drink very often, maybe I'm keeping them too busy.

It might also be that I have a lot of other joy and mood boosts. If beer is your only joy source and everyone's depressed all the time, social drugs may mean they drink a lot to stay happy.

I have noticed animals get cirrhosis with even a "small" alcohol tolerance.

You mean in real life? Definitely not this.

I set my colonists to "No Drugs" unless they have a chemical interest / fascination. I don't like the plebs drinking up my profits.

Colonists that have chemical interest are restricted to smoke leaf.  It took roughly two years before they developed any negative effects.

Keeping folks happy is relatively simple.  Give them a nice room, a horse shoe pit and a pool table. No troubles.
