For almost 1.5 year running colony no one get sick for Malaria, Flu, gut worms, etc or have wound infection. Maybe I just too lucky, but I also try to generate Disease Incident in developer mode. It possible to infect pawns with Mechanites, or muscule parasites, but still not possible to infect with Flu or Malaria (i not try the whole list of diseases). No one is affected by penoxyciline, to be clear. Not sure about rare diseases like carcinoma.

are you playing standart scenario or playing with mods?
diseases may be disabled if this one is custom scenario ar if you using mods, that would disable them.

No mods. I played "Lost Tribe" scenario in Desert biome long enough (Cassandra Intense), also played about 5 minutes in "Crashlanded" in tropical biome (Randy Extreme). No one get Flu after I 10 times clicked on "Execute incident - Disease_Flu", in developer mode. But maybe there is a hidden period in disease progress? Because a Mechanites, gut worms or muscule parasites reported immidiately after "Execute incident" clicked.

Ark

Flu indeed has a hidden period. Check the dev options on the pawn's health tab.
After 73 days in a tropical map Randy hit me with malaria and sleeping sickness at the same time, diseases work.

My colonists have gotten the plague before, but I haven't gotten a single infection yet, so I can say this isn't just blind luck.

Wow! After 2.5 years running colony, very first incident of plague happens. So my guess about there is no diseases at all is incorrect. But probability of disease incident seems is very low.
