Also when I was playing it wasnt made clear that no hoppers = starve to death.

Could be because you're using simple doors? Guess it makes sence that powered doors are more likely to lock people in

This is a bug. But you can avoid it by not having your prisoner cells have zero standable space. Leave a square or two to stand beside the bed.

I have a larger prisoner room and had a doorway issue with prisoners as well.

In my case, I convinced a prisoner (Yumi) to join me.  I then realized I didn't want him, so I had another colonist arrest him.
The arresting colonist dropped Yumi in the doorway and it started spamming that Yumi was escaping.
Then my warden would get stuck trying to chat Yumi up.  And I'd get messages that a colonist was starving, but it was the arrested and "escaping" prisoner (Yumi) that was starving.  I could also still assign tasks to Yumi via the overview.

Then I sold Yumi to slavers and he still appeared on my overview and as starving, but his graphic no longer appeared in the game.  Clicking on him in the overview to center on him took me to the doorway.

I saved and loaded the game and he disappeared, like everything was fine the whole time.

Good report. I also narrowed it down to dropping prisoners in a doorway. My earlier explanation about the tiny room was an older bug that I fixed in A2 (I thought it came back).

So, Tynan, you're saying it's never good to drop anything in any sort of portal between realms and that this rule does not exclude prisoners at doors between jail and the outside world.
I like how this game can result in quotes that would be quite unnerving when said in public, out of context. - Myself

I have reproduced this on a colony.

1) Savegame with a normal colony.  To reproduce from here, draft any colonist and have him arrest any other colonist.  The prisoner will be dropped in the top right doorway of the large prison.  https://dl.dropboxusercontent.com/u/1043580/Freefall%203.rim

2) Savegame where a colonist is arresting another colonist.  Colonist "The Nose" is carrying a prisoner.  You can see that his path ends at the doorway to the prison.  Upon arrival, the bug will happen.  https://dl.dropboxusercontent.com/u/1043580/Freefall%203%20-%20about%20to%20bug.rim

This colony was created in 413 on windows.  No mods ever used nor any cheating.
