Insta-kill heart attack?

According to the wiki, heart attack always starts at 40%. But I have this guy who started off his heart attack with 86% and then few seconds later he just died.

He did have another heart attack before is that why this happened?

I feel like heart attacks are 50/50 as to whether or not they live. I've seen the same person have multiple and live and seen a healthy person just die from the first one.  Shitty luck is what that turns into.
