Schedule surgery

I can't figure out how to get a doctor/patient to perform a surgical procedure.
This information SHOULD BE accessible through the "doctor" page on the wiki, but is not, and it has vexed me.
I shouldn't have to make this post.
How does one schedule a surgical procedure?

Wiki is wiki and rarely uptodate.

- open the Work tab and assign a pawn to doctor.
- need at last one bed/sleeping spot marked for medical.
- select the patient
- select the health tab
- you got 2 tab, overview and Operations, select Operations.
- use Add bill for the wanted surgery.
- wait that the patient lay at the medical bed, or force him to do this.
- wait for the doctor to do the surgery or force him to do the operation

When the doctor don't start to do the surgery
- check if the patient is allowed to use (herbal) medicin and you have that and the doctor isn't restricted to a zone and can't reach it.
