This idea came to me after looking at Gastronomy:

quests that have a guest (or straight-up guests from Hospitality) who want you to operate on them to attach that new shiny archotech leg, remove a nasty carcinoma or cure some other ailment.

High-grade mods like archotech or advanced bionics would be provided, given you are unable to craft them. Lower grade stuff has a chance for you to make them beforehand, which gives you a bigger payout.

Failing the surgery makes you not get the rewards and the guest leave (given they are still able to) once they are not anymore anesthetized, and depending on whether the failure was minor or major, you anger the faction of the guest lesser or to a higher extend. If you are still able to provide the operation (let's say you still are able to attach the/another new arm of equal or greater quality) the reputation loss would still occur, but to a lesser extent, and you'd still get your rewards.


They could also arrive with another person that, in case you fail the operation, straight up call a shuttle and take them with them so you can't retry (or only have as long as the shuttle takes to arrive to start again).
