Rescue shuttle doesn't arrive

Accepted a crashed shuttle quest. Fought off the assailants and saved the Commander as required. Rescue shuttle countdown happens and I get the notification that the shuttle has landed but there is no rescue shuttle at the location. Waited to see what else would happen and when the shuttle timer reaches zero, the quest fails. Commander is still wandering around by his damaged shuttle...
