I find the current disease system pretty dull as there isn't much variation between the diseases except for different effects they have on particular areas of human body. My idea would be to make diseases a bit more of an issue by making them contagious. This would add more variation in treating the diseases as it might lead to isolating ill colonists from others. It would also make penoxycyline have more use as I personally find it useless due to the fact that the diseases appear randomly like many other events.

I think the point of penoxycyline is to ensure that your doctors are less likely to get ill, not the whole colonie. So when in need, they can care for other pawns.

I see some problems with contagious diseases. How do you handle them? If you have a doctor which treat the pawn will the doc get the disease too? Who treats him then?
Atm you normaly afford one hospital. But if you want to isolate the pawns, i would send them in there own rooms. Case closed. And if the doc can get ill and the illness could be deadly...yeah than they go to die.
Imagen a colonie in year 1. They dont have many rooms or doctors or medication.

Also you can not scale it. Like raids get bigger, ships drop more mechs. But one pawn with a disease could infect everybody. There isnt a difference in how strong your colonie is going.

Perhaps diseases could be lurking in the shadows, like when one pawn is hit you have to check everybody if the have also a disease. But by writing it, it also sounds dull. Just more work.

ATM i dont see a fitting way for it, but perhaps someone has an idear.
