How do I input my name for Name in Game?

I purchased name in game access two days ago on Steam, but do not know how to actually go about using it.  Any help would be appreciated.

Take a look at the Creative Reward Guidelines.
There you'll find the link to the page for the Creative Rewards Login.
Go there, register yourself (with the email address you've used to buy it), confirm your email, login and it should give you access to the form to enter your name.
