What is the Worst Incident Which Involved a Colonist Misfiring?

What is the worst incident which involved a colonist misfiring their gun that has happened to you? Mine will have to be:

My best handler tries to tame the ostrich that lives inside my walls.
The ostrich goes berserk and my handler and his Yorkie run to where my shooting skill 6 pawn is.
With a Yorkie, my handler surprisingly manage to hold his own while my other pawn gets a sniper gun.
As I watch my handler hold off the ostrich, I begin to hope that the other pawn's shot will hit the ostrich.
After missing his first shot, he then shoots my handler's right hand off.
Then I watch in horror, the other pawn then shoots my Yorkie in the head, who was still helping hold off the ostrich.
The handler, after losing his right hand and his dog, begins to lose the fight (no duh).
I think to myself that this cannot get any worse.
The other pawn after missing a bunch of times, then shoots my handler in the head, killing him.
As I facepalm, the ostrich ran up to my other pawn and easily kills him.

The other pawn never even hit the ostrich with the gun.
I rage quited.

Yes, a pawn shooting at a target that other pawns are meleeing will always hit the other pawns.

Don't grab a rifle.  Grab a plasteel longsword.  And grab everyone else at the base and have them punch the ostrich until it dies.

I had a hunter with a sniper rifle take off the head of a hauler that happened to be walking by the target.

Colonist's hunting, one of them somehow kills another colonist by hitting their brain...

50+ animals

i get an infestation in middle of a huge caravan visiting me. you can guess where this is going...

my guys tarts fighting the infestation and a visitor gets 1 shotted. inmediate revenge from the 20+ man caravan,

end result? 5 dead animals on my side and the timber wolf freezer full of juicy visitor corpses. and a pissed off faction that has failed for the next years to get their revenge.

not a misfire, persay, but a tragic case of excessive force

When someone berserked, a dude threw a molotov...and hit him in the face

The fire burned out an eye, and ear, and scarred the head

The best part is, someone who the guy hated, made art about the time he got a molotov to the face, which everyone thought was beautiful. So essentially a horrifically maimed dude became a meme, for being horrifically maimed.
